Water has arrived faster than the mode can dose it for long enough that its Max Pending queue is full. Volume beyond the queue is written off — so a surge cannot trigger a long overfeed later — but the feed is now under-delivering.
Keeps dosing flat out. Nothing is blocking the mode.
